Irina Koyfman, NP
Irina is a Nurse Practitioner who serves as the Director of Transitional Care at Maxim Healthcare. Her 15 years of clinical experience combined with her strong background in executive leadership and management make her the natural choice to lead University of Maryland St. Joseph Medical Center’s newest initiative called Maxim Transition Assist.
Her expertise in coordinating clinical, psychosocial and post-acute care activities to provide at-risk patients with the highest levels of care and support they need during the critical 30-day post-hospital discharge period when at-risk patients are most susceptible to readmission.
Irina is currently pursuing doctorate degree in nursing with the focus on community health and transitional care. She enjoys cooking, reading, playing piano and spending time with her family, which includes husband Greg, daughters Jana and Sonia, a pug and a cockapoo.
